We would like to ask for your cooperation.

In case of sickness, please do not accept the official "Incapacity for work/ sick leave certificate" (in Czech "Pracovní neschopnost" or slang. "neschopenka") from your doctor. (see the picture attached)

This sick-leave certificate is accepted primarily by Czech employers (companies and institutions).   As you know, all of you have got your private health insurance cover which is not compatible with this one. If you need to ask for a sick note due to illness, please always use the attached document named Long term illness confirmation.
